Referrals

Referrals will be for families with at least one child under the age of five and can be either of the following:

  • a parent or child with a disability
  • multiple birth children (i.e. twins)

and who would like support from a trained volunteer parent.

We also take referrals for our new ‘Enjoy and Achieve’ Project – supporting parents to become involved in their child’s learning and development through play and having fun!

Referrals should be made by telephone to the Home-Start office (01795 429007).  The referrals will only be accepted if you have the parent’s permission to request our service.  A telephone only referral system ensures we only accept referrals that meet our criteria.  We can also inform you immediately of volunteer availability or if we have a waiting list.

We can also accept self-referrals from these families.  The Home-Start support for families with multiple births can be through our Twins Group, and/or home visits from a volunteer.  The support can be long term up until the children go to school.

How to refer a family

You should:

  • Ensure the family comes under the criteria specified above
  • Discuss Home-Start support with the family – they MUST agree to receive the offer of Home-Start support
  • Contact the Co-ordinator who will discuss the referral with you by telephone.

We will:

  • Set up an initial visit with the family to discuss their needs and tell them all about Home-Start
  • Let you know when Home-Start support starts and ends
  • Keep you informed, within confidentiality guidelines, about progress and outcomes.

How to refer a twins/multiple birth family

You should:

  • Ensure the family have twins up to the age of 5*/ are expecting twins
  • Discuss Home-Start support with the family – they MUST agree to receive the offer of Home-Start support
  • Contact the Co-ordinator, who will discuss the referral with you by telephone.
  • *twins up to age of 5 for home-support ONLY, not Twins Group (for 0-4’s)

We will:

  • Set up an initial visit to the family to discuss their needs and tell them all about Home-Start
  • Give them details about our Twins Group – they can attend this group AND receive home support if they wish
  • Carefully match them with a volunteer who will visit the family at home once a week
  • Let you know when Home-Start support starts and ends
  • Keep you informed, within confidentiality guidelines, about progress and outcomes.
